Occupational Therapist OT Home Health (PRN)
About The Position
Empath Home Health, a member of Empath Health, is seeking a compassionate PRN Home Health OT (Occupational Therapist) to work with our Home Health team to deliver individualized care to patients in their homes in east Pasco County (Wesley Chapel, Odessa, Zephyrhills, Land O' Lakes, and Dade City). They provide skilled nursing and therapy services to help patients recover and maintain independence at home, along with personal care support. This is a PRN role with 5-10+ home health visits per week, focused on meaningful, patient-centered care in the comfort of patients' homes. Empath Health is a not-for-profit healthcare organization providing Full Life Care through a connected network of services across Florida.
Requirements
- Active Florida OT license
- Minimum of 1 year experience as a licensed Occupational Therapist (OT)
- Strong assessment, critical thinking, and patient education skills
Nice To Haves
- Home health care experience strongly preferred
- Prefers experience using Homecare Homebase (HCHB) EMR
Responsibilities
- Perform comprehensive patient assessments, including OASIS assessments at required intervals
- Develop, initiate, and revise individualized plans of care as patient needs evolve
- Deliver skilled occupational therapy services and treatments as outlined in the plan of care
- Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies
- Collaborate with the interdisciplinary team to ensure seamless, holistic care
