Occupational Therapist (OT) or Certified Occupational Therapy Assistant (COTA) – Sioux City, IA Blue Stone Therapy is seeking an Occupational Therapist or an Occupational Therapy Assistant to provide rehabilitation services to adult and geriatric patients at Westwood Specialty Care in Panora, IA. This role focuses on evaluating and treating adult and geriatric patients in a skilled nursing and post-acute rehabilitation setting. The OT or COTA will work as part of an interdisciplinary rehabilitation team to improve activities of daily living, functional independence, upper extremity strength and coordination, cognitive function, and overall quality of life. What You’ll Do – These will be different based on the position Provide skilled occupational therapy services to adult and geriatric patients in a skilled nursing and post-acute rehabilitation setting Perform patient evaluations and develop plans of care (OT) or implement treatment programs under the direction of a supervising Occupational Therapist (COTA) Deliver therapy focused on activities of daily living (ADLs), upper extremity function, coordination, cognitive skills, and safety awareness Monitor patient response to treatment and adjust interventions in collaboration with the therapy team Maintain appropriate patient schedules per established guidelines Complete timely and accurate documentation to support quality care and reimbursement requirements Collaborate with interdisciplinary team members and facility staff to support positive patient outcomes This position will serve multiple partner communities, including Accura HealthCare of Le Mars and Holy Spirit Retirement Home with travel between locations as needed. What You’ll Do Perform occupational therapy evaluations and develop individualized plans of care Provide skilled occupational therapy treatment focused on functional and measurable outcomes Address ADLs, functional mobility, cognition, strength, positioning, and discharge planning Supervise Occupational Therapy Assistants in accordance with regulatory and company standards Maintain appropriate patient schedules per established guidelines Complete timely, accurate documentation to support quality care and reimbursement requirements Collaborate with interdisciplinary team members and facility staff to support positive patient outcomes

Requirements

  • Graduate of an accredited Occupational Therapy or Occupational Therapy Assistant program
  • Current OT or COTA license (or eligibility for licensure) in the applicable state
  • Experience in skilled nursing, long-term care, or post-acute rehabilitation preferred
  • Strong clinical reasoning, communication, and organizational skills
  • Commitment to patient-centered care and professional standards
