About The Position

Sage Group HealthCare Staffing is seeking a Home Health Occupational Therapist for a 13-week travel/contract position in Alpena, MI. This full-time role involves providing occupational therapy services to patients in their homes, consulting with physicians and interdisciplinary teams, and developing treatment plans to maximize patient independence. The therapist will utilize skills in evaluation and treatment, professional consultations, and in-home OT programs to assist patients in restoring function, relieving pain, and preventing disability, all while helping them maintain dignity and independence in their homes. The role requires driving to patient homes within assigned territories daily.

Requirements

• Current Occupational Therapist experience: 1-2 years
• Recent/Current Home Health OT experience: 12 months within the past 3 years
• Home Care Home Base (HCHB) charting experience
• Must have valid Driver’s License and Personal Auto Insurance
• Must be Covid Vaccinated or provide medical or religious exemption
• Bachelor of Science degree in Occupational Therapy or Similar
• Current State License as Occupational Therapist for MI
• BLS Certification from AHA or American Red Cross

Responsibilities

• Provide Occupational Therapy services to patients/clients at their homes.
• Consult with the physician and an interdisciplinary team in the development of a plan of treatment.
• Utilize skills in patient/client evaluation and treatment, professional consultations to achieve maximum patient/client independence in accordance with the individual's functional level and potential.
• Plans and prepares treatment plan based on evaluation of patient data.
• Provide in-home OT treatment programs to assist patients in restoring function, relieving pain, and preventing disability.
• Allow patients and their families to maintain dignity and independence while receiving safe, effective rehabilitation in the security of their home.
• Drive to patient homes in the assigned territories on daily basis.
