Occupational Therapist Assistant COTA for Home Health
About The Position
FeldCare Connects is seeking a self-motivated Occupational Therapist Assistant (COTA) for Home Health. This role involves delivering premier excellence of care to adults of all ages. The position is for an Independent Contractor serving Santa Clarita and surrounding areas. The Occupational Therapist in Home Health is responsible for assessing and evaluating patient care needs related to functional status, activities of daily living, fine motor coordination, home assessments, adaptive equipment, and other occupational therapy needs based on medical conditions. Based on this assessment, the Occupational Therapist determines a treatment plan, performs interventions to improve patient wellbeing, and evaluates progress.
Requirements
- Ability and enthusiasm in working with all ages
- Occupational Therapist license and registration by the state
- Completion of an accredited Occupational Therapist program

Responsibilities
- Provide services defined by state laws governing the practice of Occupational Therapy, in accordance with the plan of care, and in coordination with other health care team members.
- Lead the patient in the use of therapeutic, creative, and self-care activities to improve functioning and safety.
- Observe, record, and report the patient's response to treatment and changes to the patient's condition.
- Instruct patients, families, caregivers, and the health care team in areas of therapy where they can assist the patient.
