About The Position

Adaptive Home Health is building a higher-acuity, patient-centered home health model across Texas. Our ultimate mission is to dramatically improve patient access to home health care. The COTA role is the front line of that mission: you deliver high-quality functional visits in the home under the OT plan of care, helping patients stay safe and independent. We build technology to better support our field clinicians faster and more supported. Ultimately, our clinicians are the customers of our engineering team. Our charting product is designed to reduce documentation burden and keep visits moving, and our scheduling product will make it easier to coordinate visits, reduce last-minute changes, and protect clinician time.

Requirements

• Active Texas COTA license (in good standing)
• Strong clinical judgment and comfort working independently in the field (within COTA scope and under OT supervision)
• Strong communication and documentation
• Current driver’s license and comfort with daily travel across assigned territory

• EMR Experience

Responsibilities

• Deliver skilled COTA visits in the home under the supervising OT’s plan of care.
• Execute functional treatment plans: ADLs/IADLs, safety/fall prevention, energy conservation, UE function, adaptive equipment training.
• Track objective functional progress and communicate updates/concerns to the supervising OT.
• Coordinate with nursing and the broader care team: escalate changes in condition and safety concerns.
• Document promptly and thoroughly.
