About The Position

Synergy Care is hiring a full-time Occupational Therapist Assistant/Rehab Director for an outpatient clinic serving an AL/IL community in Fort Worth, TX. You must be a licensed therapist. We are looking for a team-oriented clinician who is passionate about providing quality patient care and making a difference in the lives of others.

Requirements

• Graduate of an ACOTE-accredited Occupational Therapy Assistant program (or graduating within 6 months)
• Current NBCOT certification (COTA) required
• Active Louisiana OTA license (or eligible for licensure)
• Strong communication and teamwork skills
• Commitment to patient-centered care

Responsibilities

• Evaluate patients and develop individualized treatment plans
• Provide ongoing treatment and discharge planning
• Supervise assistants and aides
• Participate in case management
• Deliver high-quality occupational therapy services in accordance with state regulations and Synergy Care guidelines
• Represent Synergy Care’s commitment to excellence in every patient and facility interaction
