About The Position

The Clinical Nurse Navigator (CNN) is a professional role model and clinical expert who serves as an advocate and educator responsible for facilitating patients’ and/or caregivers’ experience by guiding patients across the continuum, providing education and support, and helping predict follow-up care needed for treatment and disease process in order to promote medical optimization to ensure best outcomes and best value of care.

Requirements

• Require one (1) to three (3) years of experience in a specialty area.
• Require graduate of an accredited school of nursing or a diploma program.
• For new hires after 2012, Bachelor of Science Degree in nursing (BSN) is required within specified timeframe as outlined in the New Hire BSN Agreement policy (126.045).
• BLS: Basic Life Support/CPR
• FDL: Valid Driver's License
• FL RN: FL Registered Nurse License
• As part of Sarasota Memorial Health Care System’s commitment to keeping people safe, all individuals providing care to vulnerable populations are required to undergo background screening through The Florida Care Provider Background Screening Clearinghouse.
