About The Position

Provides nutrition assessment, counseling, education, and referral services to clients while evaluating nutritional needs, health risks, and eligibility requirements. Maintains accurate documentation, monitors program compliance and quality improvement efforts, and supports clinical audits and accreditation requirements. Collaborates with healthcare providers, clients, and community partners to coordinate services and ensure appropriate nutrition care and resources.

Requirements

  • Bachelor’s or master’s degree in Nutritional Sciences, Community Nutrition, Clinical Nutrition, Dietetics, Public Health Nutrition, or Home Economics with emphasis in Nutrition.
  • 15 hours of nutrition education.
  • Over 18 years of age.
  • Possess a valid driver’s license.
  • Current CPR certification.
  • Knowledge of electronic documentation program.
  • Knowledge and ability to monitor and measure program compliance with all applicable policies, funding and billing regulations and laws.
  • Knowledge of compliance audits and ability to participate in audits related to documentation requirements, clinical quality, and program licensing regulations, and accurately report findings.
  • Knowledge and ability to monitor all aspects of quality assurance/improvement and to implement plans to further improve quality.
  • Knowledge and ability to assist in monitoring program’s compliance to applicable accreditation standards and implementing corrective action plans as necessary.
  • Knowledgeable and detailed operation of the Louisiana software for warrants preparation, distribution and records maintenance.
  • Ability to complete health measurements on all applicants (height, weight, blood samples) following approved methods.
  • Ability to perform clerical duties including the use of a copier, fax, computers and other office equipment, filing and web based operations and light housekeeping duties.
  • Maintain a clear driving record.

Responsibilities

  • Screens applicants for eligibility by interviewing them to determine and verify household income, and computing the information using electronic software.
  • Provides participant-centered nutrition counseling and referral services for clients.
  • Completes a nutrition assessment for applicants, accurately assessing nutrition risk and providing nutrition and health recommendations.
  • Promotes breastfeeding for clients, community partners, and health providers, referring breastfeeding clients to the Breast Feeding Peer Counselor Program.
  • Prescribes food packages tailored to participant needs and issues warrants following policies and procedures.
  • Schedules and maintains appointments in a computerized database.
  • Refers high-risk clients to Registered Dietitian services for additional nutrition counseling.
  • Completes health measurements on all applicants, including height, weight, and blood samples, following approved methods.
  • Performs clerical duties including the use of office equipment, filing, and web-based operations.
  • Maintains a clear driving record for driving company vehicles when necessary.
  • Attends all scheduled staff meetings.
  • Performs other duties as assigned.
