About The Position

Healthy Lincoln County is seeking a full-time Nutrition Education Associate to support community-based nutrition education and Food Is Medicine strategies in Lincoln County and across Maine. This position is part of the Rural Health Transformation Program (RHTP), a five-year initiative designed to improve population health by connecting healthcare, nutrition, and community resources to address the root causes of chronic disease. This role focuses on expanding access to evidence-based nutrition education while strengthening systems that make healthy food more accessible and integrated into healthcare and community settings. The position works closely with schools, healthcare providers, and community organizations to build sustainable approaches that support prevention and long-term health. Healthy Lincoln County’s work is rooted in the belief that food access and public health are deeply connected. This position supports that vision by helping implement programs that meet people where they are, through education, partnerships, and practical strategies that improve everyday access to nutritious food.

Requirements

  • Bachelor’s degree in nutrition, public health, health education, behavioral sciences, or a related field (or equivalent experience)
  • Experience building or maintaining community partnerships across diverse settings.
  • Ability to work independently while managing multiple responsibilities in a dynamic environment
  • Strong organizational, communication, and interpersonal skills
  • Proficiency with Microsoft Office, including Word and Excel
  • Flexibility to adapt to changing program needs and priorities
  • Valid driver’s license and reliable transportation is required
  • Ability to lift 30–40 lbs
  • Background check required

Responsibilities

  • Deliver evidence-based nutrition education programs focused on chronic disease prevention and management.
  • Implement Food Is Medicine approaches such as medically tailored groceries, food pharmacies/pantries, produce prescription programs (FVRx), farmers market incentives, gleaning, and school-based strategies.
  • Build and maintain partnerships with community organizations, schools, and healthcare providers to support program delivery and sustainability.
  • Conduct outreach to increase awareness of and participation in nutrition education and Food Is Medicine programs.
  • Coordinate with healthcare partners, including hospitals, primary care practices, and Federally Qualified Health Centers, to support referrals and enrollment.
  • Adapt programming to meet local community needs while maintaining alignment with evidence-based models and program guidance.
  • Maintain accurate records and complete required reporting and evaluation processes in alignment with CMS, Maine DHHS, and program partners.
  • Participate in trainings, meetings, travel throughout the service area, and perform other duties as needed to support Healthy Lincoln County’s public health, prevention, and food access work.
