About The Position

This role involves providing care and support to residents in an assisted living (AL) or skilled nursing unit (SNU). Key responsibilities include adhering to a philosophy of dignity, privacy, independence, choice, and individuality for residents. The position requires assisting with coordinating ancillary health care and health education services, ensuring accurate and thorough documentation of service and medication delivery, and maintaining regulatory compliance. The LPN will act as a resident advocate, promote infection control, track trends for Quality Management Review, and assist in verifying physician orders and resident information. The nighttime HW Coordinator must be immediately available and awake.

Requirements

Current license as a LPN/LVN or RN (as required by state guidelines) to include annual continuing education requirements.
Must be a licensed LPN/LVN or RN (as required by state guidelines) and any necessary examination(s) in accordance with laws of this State.
Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served.
Must be a supportive team member, contribute to and be an example of teamwork and team concept.
Must possess the ability to make independent decisions when circumstances warrant such action.
Must have patience, tact, a cheerful disposition, and enthusiasm, as well as the willingness to handle difficult residents.
Must be willing to seek out new methods and principles and be willing to incorporate them into existing nursing practices.

Nice To Haves

Prior experience in AL or SNU.
Prior experience as a LPN/LVN or RN for at least 1 year.

Responsibilities

Adhere to and convey a philosophy of supporting dignity, privacy, independence, choice, and individuality for Residents.
Assist current HWD in coordinating, in cooperation with the Executive Director, ancillary health care and health education services for Residents.
Aid that all required documentation is completed accurately and thoroughly, to include service and medication delivery, and that all records required by regulatory agencies or company policy are maintained effectively and according to guidelines.
Act as Resident advocate in medical/social situations involving Resident and/or designated agents, physician, or other agencies under the direction of the current HWD.
Promote infection control procedures to be known and followed by all Associates.
Track trends for Quality Management Review.
Assist in verifying physician orders, move-in/move-out information and initiate appropriate action and follow-up.
Assist in identifying Resident problems, concerns, or issues and providing appropriate follow-up to ensure resolution.
Be immediately available and awake as the Nighttime HW Coordinator.
