About The Position

The Nursing Case Manager develops and implements care plans that coordinate clients’ home care services and follow physicians’ orders. The Case Manager ensures that services are delivered according to licensing guidelines, professional medical standards, and agency policies and procedures. The role includes client intake and assessment, care planning, monitoring service effectiveness, supporting and supervising aides, educating clients and caregivers, and coordinating care and medical documentation. Travel is required as needed.

Requirements

• Bachelor’s degree in Nursing.
• At least two years of experience.
• Valid nursing license in the state of work.
• Travel as needed.

Responsibilities

• Perform client intake assessments, including submitting required documentation for prior authorization of client services when needed based on clients’ pay sources.
• Develop plans of care based on nursing diagnoses and input from clients and physicians, including medical interventions and measurable goals or outcomes.
• Routinely review pertinent client medical data to determine whether services are helping clients reach their maximum rehabilitation potential.
• Investigate and take appropriate action on client or employee concerns and complaints.
• Ensure aides are properly oriented and trained to meet client needs in accordance with agency policies and procedures.
• Conduct periodic supervisory visits for aides and provide written evaluations regarding service delivery.
• Instruct clients, family members, and primary caregivers as needed to help maintain or achieve each client’s optimal level of functioning.
• Coordinate client home care services by completing required documentation on time and transferring pertinent medical data promptly to physicians, therapists, and other caregivers.
• Provide input on the formulation or modification of agency policies, procedures, and practices related to client services.
• Participate on the advisory board or agency quality-assurance committees when requested.
• Perform other duties and actions as required.
