About The Position

The Community Living Associate (CLA) provides supportive services to participants (residents) in community living settings, during transportation and appointments, and in the day health center. The CLA works under the management of the PACE SEMI Community Living Manager and in coordination with the Lead CLA, and may be supervised by the manager, a registered nurse, or the Lead CLA. Services are provided according to each participant’s care plan, which is based on initial and ongoing assessments by the manager, assigned RNCM or clinical team, and Lead CLA. The CLA helps maintain participants’ physical and emotional comfort, supports their independence and self-reliance, and contributes to a nurturing environment consistent with PACE SEMI’s culture, mission, values, and Ten Principles. The CLA documents care promptly and accurately, keeps supervisors and the clinical team informed of participant progress, reports complaints and schedule deviations, and responds to emergencies according to established procedures.
