About The Position

Ebenezer Integrated Care & Rehab (EICR) is seeking a Nursing Assistant (CNA/NAR) for its newest Skilled Nursing Community in downtown St. Paul, Minnesota. This role is responsible for providing resident care and services in transitional care units and long-term care settings, including memory care. EICR is part of Fairview's Community Health and Wellness Hub, committed to delivering innovative, collaborative care for seniors with long-term, transitional, and dementia care needs.

Requirements

  • Less than 1 year of experience.
  • Nursing Assistant Registered on the MDH Nursing Assistant Registry by start date.
  • Alternatively, individuals hired into Ebenezer’s Nursing Assistant Course or Ebenezer’s Pre-CNA job for Nursing Assistants will be granted a 4-month post-hire period to become active on the Minnesota Nursing Assistant Registry.

Responsibilities

  • Assists residents with personal cares based on individual plan of care, including AM/PM cares, weekly baths/showers, oral hygiene, nail care, shaving, and ensuring residents are clean, dry, and dressed appropriately.
  • Provides offloading for residents and performs Restorative Nursing Programs per facility policy.
  • Provides adequate nutrition & hydration by distributing meal trays, assisting residents, documenting intake/output, and providing nourishment and water between meals as ordered.
  • Assures individual toileting plan is followed and answers call lights promptly.
  • Observes residents and reports status to appropriate staff, receiving report at the start of the shift and giving report to the team leader at the end of the shift.
  • Notifies appropriate staff of any changes in a timely and appropriate manner, including incidents, skin integrity, behavior changes, and illness.
  • Offers pertinent observations and input for documentation and care plan updates, utilizing the Nursing Assistant Kardex or Care sheet.
  • Takes and documents weights as ordered and documents resident cares promptly in EHR.
  • Responds to emergency situations appropriately and handles resident and family complaints tactfully, referring concerns to appropriate staff members.
  • Follows Ebenezer policies and procedures and reports resident change of condition to Licensed Nurse.
  • Attends mandatory, annual, and other educational opportunities on site.
  • Maintains order and cleanliness in resident rooms and other work areas.
  • Reports malfunctioning equipment to the Maintenance Department.
  • Assists with admissions, transfers, and discharges as needed.
  • Answers phones with proper customer service etiquette.
  • Performs other duties as assigned and directed.
