About The Position

We are seeking a dedicated Nurse (LPN or RN) to join our team, making a difference in the lives of older adults in our 43-bed skilled nursing facility. This role involves ensuring quality care through staff monitoring, physician contact, care planning, and resident observation, while also providing direct care. The position operates on 12-hour shifts on Sundays and Mondays, with a resident caseload of 18-21 and a NAC ratio of 1:8. Support is provided by a Nurse Manager.

Requirements

  • Graduate of an approved school of licensed practical/vocational nursing.
  • Current license, in good standing, as an RN or LPN/LVN in the State.
  • One year nursing experience.
  • Devotion to standards and principles of professional nursing.

Responsibilities

  • Assist with routine daily direction and monitor the work of nursing assistants, ensuring high standards of nursing care and task completion.
  • Supervise the caregiver team.
  • Provide quality customer service to residents and families.
  • Assist in the orientation of new nurses and nursing assistants and reinforce training standards.
  • Provide explanations of nursing procedures and assistance to staff.
  • Communicate regularly with the Nurse Manager or Director of Nursing regarding observations, problem areas, resident and employee activities, concerns, and conditions.
  • Observe and evaluate residents' health, mental, and emotional conditions and communicate significant changes to appropriate staff.
  • Notify the physician and family members when indicated, according to prescribed procedures.
  • Provide direct patient care as appropriate, including procedures such as catheter placement and care, colostomy care, suction, dressings, and naso-gastric tubes.
  • Participate in report at the beginning and end of shifts to ensure proper resident information is communicated.
  • Assist in the admission, orientation, and observation of new residents, ensuring physician orders for diets, therapy, and medication are transcribed and communicated.
