About The Position

We are seeking a dedicated Nurse (LPN or RN) to join our team, making a difference in the lives of older adults in our 43-bed skilled nursing facility. This role involves ensuring quality care through staff monitoring, physician contact, care planning, and resident observation, while also providing direct care. The position operates on 12-hour shifts on Sundays and Mondays, with a resident caseload of 18-21 and a NAC ratio of 1:8. Support is provided by a Nurse Manager.

Requirements

Graduate of an approved school of licensed practical/vocational nursing.
Current license, in good standing, as an RN or LPN/LVN in the State.
One year nursing experience.
Devotion to standards and principles of professional nursing.

Responsibilities

Assist with routine daily direction and monitor the work of nursing assistants, ensuring high standards of nursing care and task completion.
Supervise the caregiver team.
Provide quality customer service to residents and families.
Assist in the orientation of new nurses and nursing assistants and reinforce training standards.
Provide explanations of nursing procedures and assistance to staff.
Communicate regularly with the Nurse Manager or Director of Nursing regarding observations, problem areas, resident and employee activities, concerns, and conditions.
Observe and evaluate residents' health, mental, and emotional conditions and communicate significant changes to appropriate staff.
Notify the physician and family members when indicated, according to prescribed procedures.
Provide direct patient care as appropriate, including procedures such as catheter placement and care, colostomy care, suction, dressings, and naso-gastric tubes.
Participate in report at the beginning and end of shifts to ensure proper resident information is communicated.
Assist in the admission, orientation, and observation of new residents, ensuring physician orders for diets, therapy, and medication are transcribed and communicated.
