About The Position

Hopewell is looking for a Casual Nurse to join our Residential and Respite Services Team, supporting individuals with developmental and physical disabilities across multiple sites in Guelph-Wellington, ON! Under the guidance of the Clinical Nurse Supervisor, the nurse works within a clinical multidisciplinary team that follows a person-centered model of care for individuals with complex developmental, physical and emotional care needs. The Nurse works alongside the Support Services Worker, Support Service Specialists and other frontline workers to support fulfilling the lives of the supported individuals.

Requirements

• Diploma in Practical Nursing or Bachelor of Science, Nursing.
• Be in good standing with the College of Nurses of Ontario.
• Must have a valid Full G class driver's license.

Responsibilities

• Performs the nursing process for the supported individuals, their families and the community, in a manner that ensures supported individuals’ dignity, respect and confidentiality.
• Provides initial and ongoing assessment and interpretation of supported individual within the framework of holistic professional nursing practice, and in accordance with the principles of person-centered care.
• Assesses and interprets information and data regarding the supported individuals’ culture and values system, lifestyle, and interest and ability to participate in their care, and other factors impacting successful treatment outcomes.
• Identifies and communicates individual needs to the interdisciplinary care team and other providers of care as required.
• Provides ongoing monitoring of individuals response to intervention; identifies deviations from expected outcomes and assesses change in supported individuals’ status.
• Assess the educational needs of their coworkers.
• Develops, coordinates and updates an individualized plan of care, based on evidence, in collaboration with the patient or client and their family, the interdisciplinary health care team and other providers of care promoting continuity of care across the continuum.
• Prioritize nursing interventions to achieve patient or client outcomes and goals.
• Develops and coordinates a comprehensive education plan to meet the supported individuals and their families’ needs, including health promotion, self-management, identifying and coordinating resources to promote continuity of care across the continuum.
• Plan training plan and educational sessions for their coworkers based on the need.
• Establishes and maintains a therapeutic relationship with supported individuals and families.
• Identifies, initiates and coordinates therapeutic interventions consistent with the legislated scope of practice and level of competence.
• Implements prescribed interventions and performs independent nursing interventions.
• Ensures the implementation, coordination and ongoing evaluation of the supported individual’s care plan.
• Consults with the interdisciplinary team and other care providers (including Behaviour Therapist, Doctor’s, Dentists etc.) to discuss changes related to individual’s status, and modifies interventions as needed.
• Interprets and provides medical information including diagnosis and treatment plans to support individuals and their family and responds to their questions and concerns.
• Provides information and instruction to assist supported individuals and families to understand health care practices and processes.
• Utilizes educational material, appropriate to the capacity of the supported individual, to assist the individuals understanding of their health care, health promotion, and/or health maintenance, and the enhancement of their individual well-being.
• Care is provided alongside Behavioural Support Workers to ensure a consistent and coordinated approach.
• Responsible for implementing instructions as directed by specialists and consultants, including medical professionals and behavioural therapists.
• Updates supported individuals’ charts/forms, monitors vital signs, performs venipuncture, administers feeding and personal care plans, and all necessary Hopewell documentation etc;
• Checks medication profile and dispenses medications to assigned residents and documents medications on the profile as given.
• Monitors supported individuals’ behaviour, reaction to treatment and responses, reporting any unusual incidents or concerns to the Clinical Nurse Manager.
• Perform health acts with appropriate training, as assigned by nursing staff including but not limited to medication administration, working with G-Tubes, trach tubes, wound care, etc;
• Demonstrates competence (knowledge/skills/attitude) in the use of non-violent physical crisis interventions, trauma-informed care and other best practices available to promote healing and recovery.
• Maintain awareness of advances in client services, government regulations:
• Possess and demonstrate knowledge of First Aid, CPR, CPI, WHMIS, Occupational Health and Safety Act, fire prevention and safety procedures and other areas as deemed necessary by the Hopewell.
• Ensure that Health and Safety, Serious Incidents/Occurrences and Behaviour Support Plans are understood, followed and reported.
• Provide hands-on training for medication administration, personal care, etc. to other team members as required.
• Immediately advise the Clinical Nurse Manager and/or management team of issues, problems, concerns regarding client care issues, staff, buildings, equipment and vehicles.
• Attend to inventory needs whether it is pharmacy supplies or medical equipment supplies.
• Provide training within and outside of Hopewell to facilitate educational seminars and training sessions as assigned.
• Attending medical/specialist’s appointments alongside supported individuals when required.
• Coordinate with any medical entity when in need of appointments and inform the team.
• Use critical thinking to determine any emergency and act proactively to prevent similar situations.
• Call 911 in case of emergency and facilitate any transfer needs.
• Utilizes data obtained from the patient or client and their family and other health care providers to evaluate the plan of care including the patient or client’s responses to interventions and achievement of expected outcomes.
• Supervises and evaluates care provided by direct care assistive and support staff.
• Uses data and evidence based on best practice to evaluate nursing activities and interventions.
• Documents complete accurate and relevant data for every patient or client according to Capital Health policies.
• Evaluate if educational goals are met post-training sessions.
• Provides a supportive environment for communications and decision making by patients or clients and their families.
• Advocates for and supports the principles of patient and family centered care, and the patient or client’s rights and well being.
• Assists patient or client and families, to become aware of issues relevant to their health care needs and provides information on resources and services within the health care system.
• Recognizes trends in health care needs within patient or client groups served and involves the patient or client in planning mutually expected outcomes where possible and appropriate.
• Demonstrates knowledge and understanding of legislation dealing with patient or clients’ rights and responsibilities in health care.
• Demonstrates knowledge of the roles, responsibilities and rights of other nurses and other health care professionals.

• Be reliable, this includes: Being prepared to start your shift as scheduled. Completing required tasks at appropriate times (during shift). Following through on the directions given by the Manager.
• Adhere to the guidelines and regulations established in the workplace, this includes: Adhering to Hopewell’s Policy and Procedures (e.g. Health and Safety Guidelines, Confidentiality etc.). Recognize, identify and report problems, contentious issues and serious incidents, including recommending solutions to the manager where appropriate. • Complete documentation and training, this includes: Completing necessary paperwork on shift, with high levels of data accuracy and attention to detail. Participating in ongoing training and development and ensuring all mandatory training requirements are met and maintained.
• Maintain healthy internal relationships, this includes: Recognizing and responding to vocalized and implied needs of residents in a timely, nurturing and professional manner, as well as working well with workplace peers/colleagues. Maintaining a positive attitude about the workplace is critical to maintain a pleasant relationship with residents and peers. Directly reporting potential/actual areas of concern about residents/peers to their team leader or manager.
• Work flexible hours within a variety of settings as required by the changing needs of the program and the people we support.
• When required, must be able to undertake lifting of people following guidelines for proper back care, lifting and transferring as required by the program.
