The registered nurse shall be responsible for patient care, under the direction of the patient’s physician and supervision of the patient care coordinator. This role involves making initial evaluation visits to patients' homes, providing specialized nursing services, treatments, and evaluations. The nurse will implement the physician's plan of treatment, observe, assess, educate, counsel, and provide nursing care. Communication with the interdisciplinary team is crucial for comprehensive patient care. The nurse will maintain patient medical records, write complete and accurate notes, and submit progress notes to the physician, promptly notifying them of any changes in the patient's condition. A nursing care plan will be developed for each patient, and support, counseling, and education will be provided to the patient's family. This role also includes supervising and evaluating Home Care Aides, attending agency case conferences and in-service programs, and potentially providing in-service education to other personnel and community health clinical education to students and professionals. The nurse will also participate in agency quality assurance programs and promote awareness of home health services.
