About The Position

We are looking for a Nurse Liver Transplant Coordinator (RN) - someone to coordinate all activities, at the highest quality of care for the transplant patient and family at a developmentally appropriate level for patients served. This includes, but is not limited to, completing pre-transplant evaluations, preparation and presentation to the medical review board, coordination of organ transplant, education of the in-patient transplant recipient including discharge teaching and planning for outpatient care and post-operative follow-up. This role involves working with surgeons, hepatologists, social workers, child life specialists, dietitians, pharmacists, and physical and occupational therapists to provide the best care possible for children, particularly those with challenging and rare cases, giving them the best chance for recovery.

Requirements

  • Bachelor’s degree in nursing, associate’s degree, or technical diploma in nursing with acceptance or current enrollment in a BSN program and signed agreement or an associate’s degree in a related field accepted by the Texas Board of Nursing for the purposes of obtaining and maintaining an RN license required or must be currently enrolled in a nursing bridge program working toward an MSN with a verified BSN-equivalency or a signed agreement.
  • 2 years Pediatric or solid organ transplant nursing experience
  • RN License by Texas Board of Nursing or Nurse License Compact
  • CPR – Basic Life Support from the American Heart Association
  • Ability to demonstrate and perform generic and unit-based/critical competencies including the specific care of age and diagnosis appropriate patients.
  • Demonstrate positive interpersonal skills.
  • Demonstrate flexibility and utilize time and resources in a cost-effective manner.
  • Excellent organization skills and self-starter.
  • 2 years ICU experience and Education Experience/Care Coordination Required

Responsibilities

  • Coordinate all activities for transplant patients and their families.
  • Complete pre-transplant evaluations.
  • Prepare and present cases to the medical review board.
  • Coordinate organ transplants.
  • Educate in-patient transplant recipients, including discharge teaching and planning for outpatient care and post-operative follow-up.
  • Identify appropriate candidates upon referral and interact with appropriate staff.
  • Coordinate with multidisciplinary teams to facilitate the evaluation process for potential transplant candidates and living donors.
  • Interpret serological results and share implications with the multidisciplinary team.
  • Obtain recipient blood type, tissue typing, and cytotoxic antibodies.
  • Educate candidate/family/legal guardian about all phases of the transplantation process.
  • Understand OPTN regulations and organ allocation.
  • Collect and evaluate medical, psychosocial, and financial data for review.
  • Prepare evaluation results for presentation at the Medical Review Board (MRB).
  • Demonstrate knowledge of and compliance with OPTN/UNOS policies and listing requirements.
  • Share necessary information about the donor organ with the transplant surgeon/physician.
  • Act as a liaison between donor and recipient transplant teams.
  • Arrange transportation of teams/organ to/from donor hospital and transplant center.
  • Provide and maintain comprehensive documentation of donor and recipient information.
  • Monitor patient progress post-transplant.
  • Act as a resource for internal and external healthcare providers.
  • Inform recipient of their voluntary option for written communication to the donor family.
  • Coordinate and ensure discharge planning for continuity of care.
  • Provide and maintain comprehensive documentation of recipient's progress and education.
  • Identify appropriate living donor candidates upon referral and interact with appropriate staff.
