About The Position

Vitality Living of Aitkin is looking for a dedicated and experienced Nurse Team Lead (RN) to join our nursing leadership team. This is a great opportunity for an RN who enjoys leadership, developing team members, coordinating resident care, and making a positive difference in the lives of residents. The Nurse Team Lead assists the Director of Nursing with coordinating the Nursing Department and maintaining a high standard of resident care. This position provides leadership, supervision, education, and support to nursing team members while helping ensure compliance with applicable regulations and facility policies. The Nurse Team Lead also coordinates resident care, monitors resident needs and changes in condition, supports individualized care planning, and works with residents, families, and the interdisciplinary team to promote quality resident care.

Requirements

  • Graduate of an accredited school of nursing.
  • Current, unrestricted Registered Nurse (RN) license in Minnesota.
  • Current CPR certification.
  • Must meet all applicable Minnesota Department of Human Services background study requirements.

Responsibilities

  • Assist the Director of Nursing with coordinating the Nursing Department and supervising nursing staff.
  • Provide leadership, coaching, education, and competency training to nursing team members.
  • Supervise Nursing Assistants, Nursing Assistants in Training, TMAs, and LPNs.
  • Complete daily rounds to monitor resident care and identify changes in condition.
  • Assess resident needs and assist with developing, implementing, and evaluating individualized care plans.
  • Collaborate with residents, families, and the interdisciplinary team to coordinate resident care.
  • Participate in care conferences, admissions, discharges, and transfers.
  • Provide direct resident care and assist with meals as needed.
  • Assist with resident assessments, MDS/RAI processes, and required documentation.
  • Investigate falls, injuries, skin tears, and other incidents and assist with developing preventative interventions.
  • Assist with QAPI, audits, quality improvement projects, and survey preparation.
  • Monitor compliance with applicable Minnesota Department of Health, Minnesota Department of Human Services, and facility requirements.
  • Support staff development, mentoring, and ongoing education.
  • Perform other duties as assigned based on facility and business needs.
  • Follow all applicable federal, state, and local laws and regulations.
  • Maintain confidentiality of resident, employee, and organizational information.
  • Follow HIPAA and other applicable privacy requirements.
  • Follow all Vitality policies and procedures.
