Requirements

At least two years of nursing or healthcare experience
Experience assessing needs
Leadership experience
Google Suite experience
Able to walk, bend, stand, kneel, and lift up to 40 pounds while conducting client visits and supporting care needs.
Demonstrates ownership, teamwork, kindness, and a commitment to continuous improvement while contributing to a positive care experience for clients and coworkers.

Responsibilities

Conduct initial client assessments and ongoing follow-up visits to evaluate care needs, monitor client well-being, and ensure services remain aligned with care goals.
Develop and update individualized care plans based on client needs, changing conditions, and feedback from clients and their families.
Provide clinical oversight and support to caregivers, helping ensure safe, effective, and compliant care delivery.
Train caregivers on client-specific care needs, safety procedures, dementia care techniques, and home care best practices.
Build strong relationships with clients, families, caregivers, physicians, and community partners to support exceptional care experiences.
Maintain accurate clinical documentation, assessments, care records, and required compliance paperwork.
Respond to client, family, and caregiver concerns with professionalism, empathy, and a solution-focused approach.
Coach caregivers through performance evaluations, onboarding, ongoing education, and corrective action when needed to support their success.
