About The Position

Reporting to the Nurse Advisor, the Specialist Nurse LTC will support research & quality improvement projects, identify evidence informed best practices, and support the development of programs, policies and resources to advance the nursing department and improve resident outcomes. The Specialist Nurse LTC will collaborate with the interdisciplinary teams across all five Centres, working closely with the Advisor Nurse LTC, the Practice, Innovation, Education Team and all Central Operations teams to achieve improved outcomes within the Seniors Services Division.

Requirements

• Registered Nurse with BScN, and current registration in good standing with the College of Nurses of Ontario
• Minimum of 5 years of progressive nursing experience in a LTC setting including experience in quality improvement, clinical practice, education, and program development
• Member of the Registered Nurses Association of Ontario (RNAO) and other organizations relevant to the role
• Thorough knowledge of the Fixing long-term care Act 2021, O. Reg. 246/22 and any applicable regulations, Ministry inspection processes, and standards relevant to nursing practice
• Knowledge of InterRAI, quality indicators, clinical outcome measures, and performance reporting used to inform quality improvement initiatives
• Experience supporting implementation and evaluation of RNAO Best Practice Guidelines and nursing quality indicators is an asset
• Experience with Point Click Care
• Knowledge and additional training in Infection Prevention and Control practices
• Excellent verbal, written, facilitation, and presentation skills, with ability to communicate complex clinical information to diverse audiences
• Ability to establish and maintain collaborative relationships with internal and external stakeholders and partners
• Effective critical thinking, analytical, problem-solving skills
• Strong consultation, coaching, and facilitation skills with ability to influence change
• Customer service oriented, with the ability to effectively work within a team environment and across all levels of the organization
• Demonstrated commitment to evidence-informed practice, nursing excellence, continuous quality improvement, and resident-centred care
• Proficiency in Microsoft Office applications and experience utilizing clinical information systems and data management tools to support analysis, reporting, and quality improvement activities
• Effective conflict management, organizational, and time management skills
• Ability to work independently, under challenging conditions, and in a rapidly changing environment
• Ability to travel to all Region of Peel Long-Term Care Centres, as required

Responsibilities

• Provide advanced nursing expertise and consultation to support evidence-informed decision-making, clinical program development, service delivery, and resident outcomes across Seniors Services.
• Provide guidance and support to the nursing team, including Supervisors of Care and nurse leaders, to promote professional nursing practice, strengthen critical thinking and accountability, and support alignment with CNO requirements, nursing standards, and best practice guidelines.
• Serve as a nursing subject matter expert by conducting practice reviews, critically appraising evidence, and translating research into clinical practice, quality improvement initiatives, policy development, and program enhancements.
• Support the clinical analysis of MLTC inspection and compliance findings, identify divisional and system-level trends, recommend corrective actions, and support implementation of sustainable quality improvement initiatives.
• Monitor critical incident system reports, analyze trends, and provide recommendations to support quality improvement strategies that enhance resident outcomes and reduce clinical risk.
• Support enhancement and optimization of the electronic health record to improve nursing documentation practices, clinical workflows, and data quality.
• Support the nursing policy review cycle, including maintaining review schedules, conducting evidence reviews, stakeholder consultation, and supporting the development, revision, and evaluation of nursing policies and procedures in alignment with best practices, legislation, standards, and organizational priorities/policies.
• Develop and maintain collaborative relationship with internal and external partners to support evidence-informed practice, knowledge exchange, and advancement of nursing programs and services.
• Collaborate with the Education, Training and orientation team to identify nursing practice gaps, develop evidence-informed educational resources, facilitate/deliver, and support implementation and evaluation of learning initiatives that improve clinical practice and resident outcomes.
• Conduct nursing practice/program audits and divisional quality reviews related to legislative requirements, nursing standards, organizational policies, and best practices.
• Analyze findings and provide recommendations to strengthen compliance, quality, and resident outcomes to support the development and implementation of corrective action plans.
• Work collaboratively with the Nurse Advisor to lead and support nursing practice improvement initiatives, program evaluations, and quality improvement projects that enhance resident care and service outcomes.
• Maintain current knowledge of applicable legislation/regulations, practice standards and innovative practices in nursing.
• Provide subject matter expertise and facilitation support to assigned peer groups as needed, including the Behaviour Support Ontario (BSO) Nurse Peer Group and Supervisors of Care Peer group by promoting evidence-informed practice, supporting annual work plans in alignment with strategic plan and divisional objectives, identifying practice improvement opportunities, and fostering consistency of nursing practice across LTC centres.
• Provide advanced nursing expertise that contributes to quality improvement initiatives aligning with divisional priorities, support the goals of the Seniors Services Strategic Plan, and strengthen resident-centred care outcomes.
• Provide nursing expertise and consultation to divisional and corporate initiatives, including accreditation activities, strategic priorities, program evaluations, quality workplans, and practice improvement initiatives.
• Provide subject matter expertise for emerging nursing priorities and perform additional responsibilities consistent with the scope of the role as assigned.
• Provide subject matter expertise in the implementation, evaluation, and sustainability of evidence-informed nursing practices, including Best Practice Guideline (BPG) initiatives, monitoring of nursing quality indicators, and dissemination of findings to support continuous quality improvement across the Seniors Services Division.
• Work collaboratively with the centres to implement initiatives related to Best Practice Spotlight Organization (BPSO).
