About The Position

Outer Cape Health Services (OCHS) is seeking a Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join their team at the Wellfleet Health Center. This full-time position offers a salary range of $32.00 - $45.00 per hour. OCHS is dedicated to providing a full spectrum of primary healthcare and supportive social services to the ten outermost towns of Cape Cod, ensuring no one is denied care due to inability to pay. Requirements

  • RN: Associate of Science in Nursing from an accredited school of nursing by AACN, BSN preferred
  • Current Registered Nurse (RN) license from the Massachusetts State Board of Registration
  • Basic Life Support (BLS) Certification
  • 1 year professional nursing experience (ED, urgent care, or ambulatory/primary care setting) or comparable experience
  • Maintain and enhance professional nursing skill and knowledge by attending continuing education programs.
  • Excellent interpersonal skills, communication skills, organizational abilities, computer experience, and the ability to work within a collaborative team setting.
  • Excellent written and verbal communication skills – bi-lingual helpful
  • Proficiency in computer skills including EPIC, Outlook, Microsoft office, OneDrive etc.
  • Excellent judgment and ability to solve problems in a timely manner.
  • Ability to adhere to strict confidentiality standards.
  • Demonstrated ability to work effectively both as a team member and independently.
  • Demonstrated ability to work in a culturally diverse and inclusive environment.
  • LPN: Certificate of graduation from a Licensed Practical Nursing program at an accredited U.S. school of nursing
  • Current and unrestricted Massachusetts LPN license
  • Basic Life Support (BLS) Certification
  • 1 year professional nursing experience (ED, urgent care, or ambulatory/primary care setting) or comparable experience

Responsibilities

  • Provide direct patient care based on the nursing process within assigned work periods.
  • Function as a member of a multi-disciplinary team to achieve patient outcomes consistent with a comprehensive plan of care.
  • Assist in triaging patient telephone calls.
  • Conduct intake on walk-ins to identify need for primary care services versus escalation to a higher level of care.
  • Support providers with patient care visits, patient procedures, patient outreach, and same day walk in care.
  • Implement orders for EKG’s, dressing changes, suture removal, ear lavages, splinting, injections, IV insertion, and IV infusions (at Provincetown Health Center location only).
  • Make appropriate referrals required for the management of patient care.
  • Obtain required patient specimens and perform or request diagnostic laboratory tests.
  • Communicate diagnostic laboratory results to provider.
  • Prioritize communication of patient’s abnormal test results to physician, and to patient (if requested).
  • Render other treatment as ordered by providers (including, but not limited to intravenous, pulse, SpO2, etc.).
  • Conduct routine patient examinations according to OCHS Nursing Protocols/standing orders (routine examinations include but are not limited to blood pressure checks, allergy injections, minor wound care, uncomplicated suture removal, etc.).
  • Clean, wrap and sterilize instruments according to policy and procedure for reprocessing of instrument.
  • Perform phlebotomy tasks when needed at the direction of clinician.
  • Participate in quality assurance/quality improvement activities and all training seminars (OSHA and others).
  • Seek appropriate and timely guidance from the Nurse Manager depending on the level of concern.
  • Provide the best patient experience possible; address patient’s concern and questions, explain procedures, testing, and diagnosis thoroughly.
  • Maintain patient privacy and confidentiality of information at all times in accordance with OCHS policies and HIPAA.
  • Maintain patient records with accurate and timely documentation in the Electronic Medical Record (EMR) in accordance with OCHS policies.
  • Educate patient in the proper use of medical equipment, prescribed regimens and other nursing care regimens.
  • Refer patient to outside sources, programs, or support groups, for additional education or referrals.
  • Promote and support teamwork by collaborating with team members including providers, Medical Assistants, and Patient Service Representatives.
  • Adhere to the National Patient Safety Goals that are applicable to ambulatory care centers, including, but not limited to, improving the accuracy of patient identification, improving the safety of using medications, reducing the risk of care-associated infections, and following the universal protocol of preventing wrong site/wrong procedure/wrong person).
  • Perform other related duties and projects, as required or assigned in order to ensure the needs of the Health Center and its patients are met.
  • Travel between OCHS sites, as needed when directed by nursing leadership.
  • Ability to work in a remote setting as needed and directed by nursing leadership.
  • Perform other related duties and projects as assigned.
