About The Position

The HealthCare Connection (THCC) is seeking a dedicated and experienced Nurse Practitioner (NP) to join their clinical team. This role involves delivering a wide range of direct primary care services, including patient assessment, diagnosis, treatment implementation, evaluation, and ongoing monitoring of care plans. The NP will collaborate with physicians and healthcare staff to provide high-quality, patient-centered care and drive positive health outcomes. THCC is Ohio’s first Federally Qualified Health Center (FQHC), founded in 1967, with a mission to provide quality and accessible primary healthcare services. They serve over 20,000 patients annually through two primary care clinics and six school-based health centers, offering integrated services including Primary Care, Pediatrics, Infectious Disease, Substance Use Treatment, Integrated Behavioral Health, Dental Services, Women’s Health, and Pharmacy. THCC is recognized as a Level 3 Patient-Centered Medical Home (PCMH).

Requirements

  • Master’s or Doctoral degree in Nursing
  • Certified Nurse Practitioner licensed in the State of Ohio
  • Minimum of 5 years of experience in family medical practice
  • Board certification as a Nurse Practitioner
  • Active DEA license (or ability to obtain)
  • Strong clinical decision-making and communication skills
  • Commitment to patient-centered care

Nice To Haves

  • FNP preferred, with a strong desire to work with children.
  • Experience in a physician office, ambulatory care, or clinical health setting preferred
  • Experience in primary care, family medicine, or community health
  • Bilingual skills (if applicable)
  • Familiarity with electronic health record (EHR) systems

Responsibilities

  • Provide high-quality patient care through the diagnosis and treatment of acute and chronic conditions
  • Obtain and review patient histories (medical, developmental, social, and family) and perform comprehensive physical exams
  • Order, perform, and interpret diagnostic tests and preventive screenings
  • Develop and manage treatment plans, including prescribing medications and educating patients on conditions and care
  • Promote preventive care through immunizations, screenings, and patient education on health, nutrition, safety, and disease prevention
  • Document all patient encounters accurately, including history, exam findings, diagnoses, and care plans
  • Review and maintain patient records, including labs, imaging, medications, and immunization status
  • Coordinate care by facilitating referrals and collaborating with healthcare team members
  • Monitor patient progress, follow up on test results, and manage high-risk patient population.
