About The Position

SOAR Community Services is seeking a Full Time Nurse Practitioner to join their Withdrawal Management and Treatment Services Team. This role is crucial in supporting individuals undergoing acute withdrawal and those in a 5-week treatment program. The Nurse Practitioner will work within a collaborative, interdisciplinary team to assess, treat, and support individuals on their recovery journey. This position also offers a unique opportunity to support client transitions back into the community through follow-up at RAAM (Rapid Access Addictions Medicine). The ideal candidate is passionate about harm reduction, trauma-informed care, and client-centered practice, and thrives in a fast-paced environment.

Requirements

  • NP (Extended Class) in good standing with the CNO
  • Bachelor of Nursing + Nurse Practitioner designation
  • Strong assessment, prescribing, and clinical decision-making skills
  • Knowledge of CNO standards and reflective practice
  • Current BCLS/CPR and required liability insurance
  • Reliable transportation

Nice To Haves

  • Experience in mental health, addictions, or concurrent disorders
  • Knowledge of addiction medicine, OAT, and harm reduction
  • Trauma-informed and recovery-oriented approach
  • Controlled substances prescribing education
  • Membership in NPAO/RNAO or CPMHN certification
  • Familiarity with relevant legislation (Mental Health Act, PHIPA)
  • Resilience in high-impact, trauma-exposed environments

Responsibilities

  • Complete comprehensive mental health, addictions, and physical assessments
  • Develop and adjust individualized care and treatment plans
  • Prescribe and manage medications, including OAT
  • Monitor withdrawal and medication response
  • Conduct medical intakes for bed-based treatment
  • Complete risk assessments and respond appropriately
  • Collaborate with internal teams and external partners (RAAM, hospitals, pharmacy)
  • Support care transitions and continuity planning
  • Provide clinical leadership, coaching, and guidance to staff
  • Contribute to program development and best practices
  • Stay current with evidence-based practice
