About The Position

The Nurse Practitioner is responsible for modeling the Compassus values of Compassion, Integrity, Excellence, Teamwork, and Innovation and for promoting the Compassus philosophy, using the 6 Pillars of Success as the foundation. S/he is responsible for upholding the Code of Ethical Conduct and for promoting positive working relationships within the company, among all departments, and all external stakeholders. If providing palliative care, the Nurse Practitioner cares for patients facing chronic, complex, and/or life-threatening conditions at various locations within the flow of patient care. S/he provides assessments, disease and treatment education, and symptom management to meet the needs of palliative care patients and their families, helping them maintain their maximum level of comfort and coping. This role involves collaboration with the PCP and specialists, ensuring compliance with practice standards and codes, and appropriate collaboration with the assigned Compassus Medical Director. The schedule must be flexible to meet agency needs. If providing hospice care, the Nurse Practitioner functions as an extension of the interdisciplinary team (IDT) and hospice physician to provide routine recertification and emergency assessments, educational, and evaluative services to meet the needs of patients and their families. S/he helps the patient and family maintain their maximum level of comfort and coping, flexes schedule to meet agency visit needs, and may be asked to provide after-hours emergency visits and/ or admissions.

Requirements

  • Master’s degree in Nursing required.
  • Minimum of one (1) year nursing practice or equivalent experience required (for Hospice Care).
  • Minimum of five (5) years nursing experience required (for Palliative Care).
  • Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
  • Ability to compute rate, ratio, and percentage.
  • Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
  • Ability to write reports, business correspondence, and procedure manuals.
  • Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties.
  • Strong written and verbal communications.
  • Ability to understand, read, write, and speak English.
  • Articulates and embraces hospice philosophy.
  • Strong organizational and time management skills.
  • Must be able to work a flexible schedule to include nights and weekends.
  • Must be able to travel.
  • Ability to maintain confidentiality of information, such as patient and Company files.
  • Active and unencumbered CNS or Nurse Practitioner license in state of employment required.
  • Certified as a Nurse Practitioner in Adult, Family, Acute, or Geriatric required.
  • Active NPI number.
  • Maintain active DEA certification if state allows prescription of controlled substances by APPs.
  • Current collaborative physician agreement in place (All states except Iowa).

Nice To Haves

  • Hospice, oncology, or home health and/or long-term care experience preferred (for Hospice Care).
  • Clinical nursing care in oncology, hospice, geriatrics, intensive care preferred (for Palliative Care).
  • Board Certification in Hospice and Palliative Care is preferred.

Responsibilities

  • Function as an extension of the interdisciplinary team (IDT) and hospice physician to provide routine recertification and emergency assessments, educational, and evaluative services (hospice care).
  • Complete routine and emergency recertification visits on each patient entering their 3rd benefit period or beyond (including admissions and transfers).
  • Complete routine and emergency assessments on each patient as indicated by departmental policy.
  • Record observations, treatments, and other pertinent information.
  • Communicate with Medical Director, Attending Physician, DCS and IDT regarding findings from visits and measures to alleviate symptoms.
  • Provide physical and emotional care to support the patient and family in period of crisis.
  • Prevent unwarranted hospitalizations by meeting patient and family needs in the home.
  • Perform routine and emergency assessments related to outpatient palliative care visits.
  • Perform bedside palliative care patient/family evaluation in the patient’s place of residence.
  • Assist with diagnosis, treatment, and management of acute and chronic health conditions.
  • Order, perform, and interpret laboratory and radiology tests within scope of professional practice.
  • Prescribe medications including controlled substances to the extent delegated and licensed.
  • Order treatments and durable medical equipment as indicated.
  • Perform other therapeutic or corrective measures as indicated, including urgent care.
  • Consult with palliative care physician or designees as needed, inform primary physician of services provided and collaborate with other physicians as needed.
  • Assist in all facets of care coordination for palliative care referrals.
  • Prepare and maintain accurate patient records, charts, and documents to support sound medical practice and reimbursement for services provided.
  • Comply with applicable laws and regulations with respect to Collaborative Agreements.
  • Initiate reimbursement for services rendered.
  • Assist patients and families with goal setting related to treatment options and advanced care planning.
  • Provide training and continuing education for staff.
  • Assist in development of clinical practice guidelines/standards in support of quality palliative care.
  • Adhere to the practice of confidentiality regarding patients, families, staff and the organization.
  • Collaborate/Communicate with IDT regarding needs of patient for clinical and psycho-social interventions.
  • Assist in identifying the need for intervention of other IDT members.
  • Effectively communicate patient and family needs to IDT.
  • Complete and submit required paperwork/clinical documentation within 24 hours of visit completion.
  • Educate patient and caregiver regarding care of patient, disease process, dying process, symptom control, wound care, goal setting, treatment options, prognosis, and advance care planning.
  • Maintain consistent communication of availability to manager (minimum 4 days/month for PPV NP).
  • Triage consults by patient acuity.
  • Complete accurate billing and coding activities for all patient encounters according to CMS guidelines.
  • Define goals for professional growth and participate actively in professional activities and organizations.
  • Engage in active and frequent self-care activities for personal and professional growth and longevity.
  • Participate in monthly IDT meeting with market team.
  • Supervise nurse trainees, including graduate and advance practice nurses in field placements.
  • Participate in supervision of other trainees, including medical students, residents, fellows, and social work and psychology trainees.
  • Perform other duties as assigned.
