We’re seeking a Nurse Practitioner who is passionate about delivering full-scope primary care and thrives in a collaborative, team-based environment. As a key clinical leader, you will provide comprehensive primary care services, including diagnosis, treatment, prevention, and care coordination. You’ll work closely with physicians and interdisciplinary team members to ensure high-quality, patient-centered care. Requirements

• Active New York State NP license and DEA certification
• Board Certified (or eligible, with certification within 18 months)
• Strong clinical judgment and commitment to evidence-based care
• Passion for community health and team-based care

Responsibilities

• Perform patient assessments, histories, and physical exams
• Diagnose conditions and develop individualized treatment plans
• Prescribe medications in accordance with NYS guidelines
• Perform minor procedures and manage chronic conditions
• Review labs, imaging, and patient communications with timely follow-up
• Coordinate referrals and continuum of care needs
• Participate in quality initiatives, outreach programs, and team collaboration
