About The Position

Centra’s PACE (Program of All-Inclusive Care for the Elderly) is a nationally recognized model of care serving adults aged 55 and older. The mission of PACE is to deliver person-centered care in the setting that’s most appropriate for each participant—whether that’s in their own home, with family, in long-term care, or assisted living. PACE is a capitated model of care, which means there are no productivity metrics, only a focus on delivering the care each participant truly needs. Everything is coordinated under one roof with a dedicated interdisciplinary team of providers, nurses, dietitians, physical and occupational therapists, social workers, activities coordinators, and transport staff. Centra Medical Group (CMG) is a comprehensive network of nearly 600 employed providers within Centra Health. Centra Health services a population of 600,000 community members with 70 different outpatient and specialty practices. Centra offers primary care and specialty services across Central and Southern Virginia. CMG includes family practices, specialists, and surgeons. CMG is also home to the Lynchburg Family Medicine Residency, a training program for future family physicians. Requirements

  • Graduate of an accredited Nursing Program, and Masters of Science Degree in Nursing (MSN)
  • Minimum of five (5+) years of progressive experience in area of specialization
  • Current licensure by the Virginia State Board of Nursing as a Nurse Practitioner with prescriptive authority.
  • Maintains BLS and ACLS Certifications.
  • Hold a current, active license as a registered nurse in Virginia or hold a current multistate licensure privilege as a registered nurse.
  • Nationally recognized certification in field of specialization

Nice To Haves

  • Experienced providers in palliative, hospice, primary care, or long-term care are encouraged to apply

Responsibilities

  • Monday – Friday 8:00am – 5:00pm – no weekends or holidays!
  • Dedicated team of providers, support staff, physical therapists, occupational therapists, dieticians, and social workers
  • 5 patients per day – small volume
  • Call is once every 7 weeks, 5pm – 8am, you are not expected to go out, calls are triaged before reaching you!
