The Nurse Practitioner (NP) is a nurse who has an advanced level of clinical nursing practice that maximizes the use of graduate educational preparation, in-depth nursing knowledge and expertise in meeting the health needs of individuals, families, groups, committees and populations. The Nurse Practitioner brings to patients a holistic approach to care, mental, physical and social. Practices independently within legislated scope of practice. • Monitors ongoing conditions for patients with stabile and chronic illnesses. • Provides family support, education and counseling on diagnosis, treatment options, evaluation outcomes and follow-up plans with a focus on health promotion, illness prevention and timely re-integration into the community. • Works with community partners in planning, treatment, discharge planning and community reintegration. • Monitors medical complications due to psychopharmacological drug use. • Supports the development of Behavioural management interventions in collaboration with the psychiatrist/physician and the health care team. • In collaboration with the Primary Clinician, assists in development of the initial care plan, based on assessment of patient needs. • Provides consultation to the treatment, community agencies and other health care providers about difficult issues, ethical practice and managing complex care needs • Integrates evidence-based research findings into clinical practice. • Participates in the dissemination and utilization of best practice guidelines.
