About The Position

MPAC Healthcare is seeking a Nurse Practitioner to support their mission of providing high-quality, patient-centered care to the senior population. This role offers an autonomous environment where providers can manage patient care, collaborate with attending physicians and other facility providers, and ultimately improve patient outcomes. The Nurse Practitioner will function as the primary provider, treating an average of 12 to 16 patients daily within a dedicated facility, building trust with facility personnel and physicians, and making high-risk decisions. The position involves identifying and handling strategic issues, fostering open communication, and adapting to the dynamic environment of a senior care facility.

Requirements

  • Nurse Practitioner License
  • ANCC or AANP Certification
  • Ability to work in the U.S.

Nice To Haves

  • Experience in a nursing home space is a plus

Responsibilities

  • Coordinate patient care with the facility Director of Nursing and attending physicians, including through proper documentation in the appropriate EMRs
  • Function as the primary provider and treat an average of 12 to 16 patients per day
  • Take initiative and collaborate with other MPAC providers to ensure patients are getting the best care possible
  • Provide care for patients in a dedicated facility by building trust and rapport with facility personnel and attending physicians
  • Feel confident making high-risk decisions in the face of ambiguity
  • Identify and handle strategic issues
  • Foster open communication and teamwork with the facility team and your Regional Clinical Manager to ensure the success of clinical programs
