STGi is seeking a Nurse Practitioner (NP) to provide services at our Community Based Outpatient Clinic (CBOC). The Nurse Practitioner is an Advanced Practice Registered Nurse responsible for providing professional clinical services under the direction of the CBOC Primary Care Provider (PCP). This role involves a wide range of health services, including urgent and emergent evaluation of episodic and chronic problems, and comprehensive, integrated primary health care. The NP will be knowledgeable in assessment, treatment, and evaluation of health problems, emergency measures, preventive health, prescribing medications, ordering and interpreting diagnostic studies, and patient/family education. The position requires maintaining current knowledge, acting as a role model and mentor, advocating for clients, and participating in professional activities. The Nurse Practitioner will work closely with the Patient Aligned Care Team (PACT) and support staff, emphasizing wellness, prevention, and early detection, while supervising support staff in patient care delivery. They will maintain universal precautions, infection control practices, and ensure a smooth patient flow in accordance with VA access standards and triage protocols. Responsibilities also include performing CPR, investigating health problems, conducting physical examinations, diagnosing health problems, developing and evaluating treatment plans, prescribing medications, making referrals, and directing healthcare services. The NP will operate therapeutic and emergency equipment, obtain patient history, perform examinations, order diagnostic tests, demonstrate knowledge of pharmacology, and administer medications within established guidelines. They will also recommend and follow up on laboratory tests, document patient information in VISTA, maintain the VISTA clinic reminder tracking system, close patient charts, and schedule appointments according to VA standards. Additionally, the NP will keep patients informed of their care plan, provide health education, assist the Registered Nurse with electronic documentation, comply with all safety and operational regulations, participate in performance improvement programs, ensure timely and accurate reporting, participate in staff development, and maintain confidentiality. The role also involves aiding in the management and coordination of the CBOC's daily activities and performing other assigned clinical duties.
