Performs wellness assessments to identify and evaluate patients’ co-morbid conditions and identifies and assists with patients’ needs including but not limited to medical, social, emotional, and financial concerns through medical record review, consultation with patient, family, and healthcare team members. Manages the care of ESRD patients, using enhanced wellness visits or CHE as a foundational history and physical, focusing on medical cost/utilization and preventing/reducing hospitalizations, and re-admissions through proactive post-discharge planning and follow-up with patients most at risk. Leads the clinical team and ensures care management and care coordination activities complement and support medical treatment plans while integrating psychosocial, and physiological needs of the patients. Documents findings in patient medical record using an Electronic Health Record (EHR) system, ensuring that appropriate diagnoses are evaluated and accurately documented. Communicates with Nephrologists, DVPs, Directors, and other NPs on clinical and operational outcomes. Collaborates with Nephrologists, other providers ICHD TMs, IKC TMs, and Medical Directors regarding the healthcare needs of the patients. Provide support for patients after normal business hours if acute clinical needs arise. Knows and complies with DaVita policies and procedures in accordance with State and Federal regulations for patient care and services (CMS, HIPAA, OSHA, or other governing agencies); Standards necessary to maintain organizational accreditation (such as NCQA); and, State or Federal regulations of Injury Prevention and Safety Training Program. Other duties as assigned (ex: participation in a QI or pilot program, serving as an active participant on a committee, creation/delivery of clinical education delivered within a DCC forum).
