About The Position

As a key member of Carolina Health Centers, Inc., the Nurse Practitioner will help establish and oversee a comprehensive, community-based primary care program to improve access to high-quality, family-centered primary care. The role involves designing and implementing an integrated care delivery model focused on prevention, health maintenance, health education, and evidence-based chronic disease management. Working with a multidisciplinary team, the Nurse Practitioner will help improve community health outcomes and promote wellness. This position offers an opportunity to contribute to the development and growth of a primary care program centered on holistic care and patient empowerment. Starting pay is $100,000.

Requirements

• Graduate of an accredited Advanced Practice Registered Nurse/Nurse Practitioner program.
• At least 2 years of experience.
• Active South Carolina medical license.
• Certified or eligible for certification in area of specialty.
• Demonstrated clinical skill.
• Ability to communicate effectively with all levels of staff.
• Critical thinking, problem-solving, and consensus-building skills.
• Ability to maintain a high level of confidentiality.
• Ability to coordinate, analyze, observe, make decisions, and meet deadlines in a detail-oriented manner.
