About The Position

Circadia Health is a growth-stage healthcare AI company focused on preventing avoidable hospitalizations and transforming senior-care operations. Its Circadia Intelligence Platform combines contactless monitoring of respiration and motion, predictive models that identify many preventable adverse events in advance, and enterprise integrations with EHR, care-coordination, billing, and compliance workflows. The technology serves more than 40,000 post-acute patients daily. Circadia Health is headquartered in El Segundo, California. The on-site Nurse Practitioner role is based in Detroit, Michigan, and combines direct patient care with integration of technology into bedside workflows. The NP assesses patients, responds to predictive health signals, and coaches facility teams to support earlier interventions and reduce preventable readmissions. The role includes hands-on training, live demonstrations, and shadowed assessments. The NP conducts initial and follow-up assessments of flagged residents, initiates or adjusts plans of care, completes consent, and coordinates with facility providers. The NP interprets trend-based insights, prioritizes high-risk patients, and supports timely interventions. The role includes efficient rounding across designated skilled nursing facilities, typically 30 or more visits per day where appropriate, and same-day documentation. The NP also shares field feedback, supports NP onboarding, and collaborates with clinical operations and product teams. Sites are generally within a 30–40-mile radius; mileage and lodging for sites outside that radius are covered per policy. Success is measured by a target of 30 patient visits per day, facility engagement of at least 80%, time to acknowledgement under 24 hours, documentation completed within 24 hours of each encounter using AI-powered tools, workflow adherence, and trends toward fewer falls and readmissions.
