About The Position

Join PrimeHealth+, a Federally Qualified Community Health Center, to be a part of our well-established primary care team located on the Western Slope of Colorado in Grand Junction! We’re seeking a Primary Care APP who values team-based integrated quality healthcare for all. Our clinics provide a full range of primary and preventive healthcare services including medical, dental, behavioral health, psychiatric, and pharmacy services. We currently have 2 integrated primary care clinics and 2 school-based health centers in Grand Junction. We have a mission-focused background with a long history of providing outreach healthcare services to people experiencing homelessness and migrant farm workers. Each of our locations welcomes patients with Medicaid, Medicare, Medicare Advantage, or commercial insurance, and those who are uninsured or underinsured. A Sliding Fee Discount Scale is available to eligible patients. Position Highlights: Position is full-time, in-person 4-day workweek with average patient volume 15-17 PPD (4x10s), with alternate schedule options available NO CALL. Outpatient care only. Integration with Behavioral Health providers on each clinic pod Robust team-based clinic team including MAs, RNs, patient navigators, case managers, and care coordinators. On-site pharmacy, dental services, psychiatry services Active Quality program with outstanding results Broad scope of practice including wellness visits, chronic disease management, and opportunities for in-office procedures based on your interests Collaborative practice including APPs and physicians with weekly Lunch & Learns

Requirements

  • Master’s degree, equivalent education, or higher degree from an accredited APN/NP program.
  • Colorado license to practice as an APN/NP without restrictions.
  • DEA license to prescribe medication.
  • Life Support certification (BLS at minimum).
  • Maintenance of Continuing Education as required by your APN/NP credentialing board and state medical board.

Nice To Haves

  • Two years of related experience as an APN/NP.
