Nurse Practioner (Physiatry APP / Physician Assistant)
About The Position
This role is for a Nurse Practitioner specializing in Physiatry APP / Physician Assistant. The day-to-day responsibilities involve rounding at 1-3 Skilled Nursing Facilities or similar rehabilitation centers, 1-3 days per week at each location. The primary focus is delivering specialized physiatry care to 25+ patients per day who are experiencing difficulty participating in therapy due to pain or functional limitations. The role requires performing focused assessments, developing individualized treatment plans in collaboration with facility primary care teams, providing targeted interventions such as joint injections, Botox treatments, dry needling, and medication management. The practitioner will serve as the sole physiatrist on-site, operating with a high level of autonomy, and will coordinate care and refer to other specialties or services as needed. This position is best suited for a self-motivated and confident clinician who thrives in independent practice environments and is passionate about improving rehabilitation outcomes.
Requirements
- Current, unrestricted PA or NP license.
- DEA licensed or willing to obtain DEA licensure.
- Comfort and interest in performing peripheral joint injections (knees, shoulders, hips).
- A genuine passion for providing professional, personalized care in an autonomous environment.
- Strong communication skills, with the ability to collaborate with multidisciplinary teams.
- Familiarity with EHR charting

- Nursing home and/or physiatry experience is a plus, but not required.
Responsibilities
- Rounding at 1-3 Skilled Nursing Facilities or similar rehabilitation centers, 1-3 days per week at each location.

- Performing focused assessments and developing individualized treatment plans in collaboration with facility primary care teams to optimize therapy participation and functional recovery.
- Providing targeted interventions such as joint injections, Botox treatments, dry needling, and medication management based on patient-specific needs.
- Serving as the sole physiatrist on-site, operating with a high level of autonomy.
- Coordinating care and referring to other specialties or services for issues beyond the scope of physiatry.
