About The Position

Silverado Palliative Coastal Valleys is hiring a Nurse Practitioner to cover San Diego County. The role provides advanced nursing care to palliative patients, supports patients and families through end-of-life journeys, and helps ensure compliance with regulatory standards. The Nurse Practitioner delivers dignified, patient-centered care and supports the operation of Silverado’s Palliative program in collaboration with an interdisciplinary team. The schedule is PRN (per diem), Monday through Friday, 8:00 a.m. to 5:00 p.m., with variable shifts as needed to meet business needs. The anticipated pay range is $75–$80 USD. Employment offers are contingent on a background check, drug screening, physical, and TB testing if applicable.

Requirements

Graduate of an accredited school of Advanced Practice Registered Nursing.
MSN or doctoral degree required.
Current Advanced Practice Registered Nurse license in the state of employment.
Complete all 8 End-of-Life Nursing Education Program (ELNEC) modules within 18 months of hire.
Valid driver’s license, good driving record, and reliable transportation.
Ability to lift up to 50 pounds.
Must clear a criminal background check, physical, and drug screening.
Comfortable working outside regular hours as needed.

Nice To Haves

Certification in Hospice and Palliative Nursing preferred.

Responsibilities

Perform comprehensive nursing assessments for palliative patients.
Assess signs and symptoms of discomfort, including pain, and take prompt, appropriate action.
Initiate and manage individualized plans of care for patients and families.
Provide direct face-to-face visits for referred and existing patients.
Communicate with physicians regarding patient updates and care plans.
Maintain knowledge of state and federal regulations and JCAHO standards, if applicable.
Participate in Quality Assessment and Performance Improvement (QAPI) programs.
Ensure services comply with hospice standards and regulatory requirements.
