About The Position

Signify Health, part of CVS Health®, is seeking a Part-time Nurse Practitioner to provide In-Home Health Evaluations. This role involves engaging with patients in their homes to bridge gaps in care and build trusted relationships. The goal is to provide flexible, comprehensive, and personalized health evaluations. The visits, lasting about 45 minutes, include a medication and medical history review, a physical evaluation, and potentially diagnostic tests. Nurse Practitioners will use an iPad with an intuitive clinical workflow and are not required to prescribe medication, order lab tests, or alter current treatment regimens. This position is part of a national network of clinicians dedicated to improving health outcomes and connecting people to the right care.

Requirements

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).

Responsibilities

• Provide In-Home Health Evaluations.
• Engage with patients in the comfort of their own homes.
• Bridge gaps in care and build trusted relationships.
• Conduct visits that include medication and medical history review, physical evaluation, and diagnostic tests if ordered.
• Use an iPad and an intuitive clinical workflow for each evaluation.
• Travel locally and within licensed states, as needed.
• Conduct virtual visits as needed.
• Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, etc.).
