Nurse Practitioner PD - Adult Infusion

University of California, San Francisco•San Francisco, CA

About The Position

The Nurse Practitioner works in UCSF’s ambulatory Cancer Center Infusion APP team, providing care across five infusion centers for oncology and non-oncology patients and at the three-bed Cancer Acute Care Clinic (CACC). The team oversees care for approximately 50–75 patients per day, depending on unit size. The position requires working in both the Infusion Center and the CACC. Infusion APPs triage acute treatment-related symptoms in real time and support continuity of care through collaboration and communication with physicians, surgeons, APP colleagues, and clinical and administrative staff. At the CACC, the APP assesses, diagnoses, and manages acute oncology symptoms that can be treated safely in an outpatient setting. When a higher level of care is needed, the APP coordinates direct hospital admission when appropriate or facilitates emergency evaluation and admission. The role includes independent outpatient encounters, including telehealth, and outpatient care coordination. The APP evaluates and manages new, follow-up, acute, routine, and specialty patients; documents care in the electronic medical record; performs authorized procedures; interprets and acts on diagnostic results; and coordinates care across the continuum. Inpatient management is not included. Advanced Practice Providers must be credentialed through UCSF’s Office of Medical Affairs and Governance before their first working day; credentialing takes 60–90 days.
