About The Position

DaVita is pioneering a new approach to primary care for patients with end-stage renal disease (ESRD) directly within our dialysis centers. We're seeking a highly motivated and innovative Nurse Practitioner to be our founding NP, helping us build and shape this groundbreaking program. This program will address the care barriers kidney patients face by bringing care to the patient within the dialysis unit and fostering seamless coordination with other critical caretakers, such as the Nephrologist, dialysis teammates, and integrated care leaders. This Nurse Practitioner will be the first hire, playing a pivotal role in defining the initiative’s clinical direction and having direct access to top DaVita leaders for feedback on innovation and care redesign. Pending performance, there will be future leadership opportunities as we scale the program.

Requirements

  • 5+ years of independent primary care practice
  • License to practice autonomously
  • DEA license and comfort in prescribing for chronic diseases typically associated with ESRD, such as congested heart failure.

Nice To Haves

  • Experience with dialysis or nephrology
  • Minimum of 5 years of clinical experience in Primary Care, Internal Medicine, or Gerontology.
  • Demonstrated expertise in the management of chronic health conditions, including but not limited to diabetes, hypertension, chronic respiratory diseases (e.g., COPD), and heart disease.
  • Proven experience in prescribing and managing medications: ability to initiate, titrate, and monitor therapies such as insulin, antihypertensive, statins, and other evidence-based treatments aligned with current clinical guidelines.

Responsibilities

  • Average 10-12 patient visits per day.
  • Coordinate patient follow-ups to proactively address patient concerns, including coordinating care with other providers, ordering key labs, and prescribing medication.
  • Build clinical relationships with nephrologists, dialysis teammates, integrated care teammates, and local PCPs and specialists.
  • Conduct annual comprehensive health evaluations.
