About The Position

The Nurse Practitioner at Oak Street Health provides effective and equitable value-based primary care to adults on Medicare, with the goal of keeping patients healthy and out of the hospital. Nurse Practitioners see patients independently and collaborate with physicians according to each patient’s needs. They provide longitudinal care and build meaningful relationships that foster trust and help patients understand and feel connected to their care plans. The role is based in an outpatient practice and works as part of a collaborative care team that includes a physician, nurse practitioner or physician assistant, medical assistants, a dedicated medical scribe, registered nurses, and care managers. Oak Street Health partners with specialists and hospitals for specialty and acute care, allowing providers to focus on primary care, care coordination, and population health within the clinic. The position reports to the Center Medical Director and works closely with operational leadership.

Requirements

Graduate of a Master of Science in Nursing, Doctor of Nursing Practice, or Nurse Practitioner Certificate Program.
National certification in at least one of the following specialties: Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner.
Active, non-probationary state Nurse Practitioner license.
Active DEA license.
US work authorization.

Nice To Haves

Primary care experience in internal medicine, geriatrics, or family medicine.
Understanding of HCC documentation, ICD-10 coding, and Health Risk Assessments.
Passion for teamwork and cross-functional collaboration.
Interest in an innovative model focused on empirically guided population health.
Bilingual proficiency in applicable areas.

Responsibilities

Provide exceptional primary care.
Conduct office visits for routine and acute issues.
Administer annual wellness visits and health risk assessments, with a holistic view of health and thoughtful, accurate, specific documentation.
Coordinate care with other providers, specialists, testing facilities, and agencies.
Lead population health efforts in coordination with the Care Team.
Assist the care team with phone triage and outreach.
Educate patients on their health conditions, care plans, and treatments.
Participate in Oak Street Health promotional activities.
Conduct home visits as needed.
Perform other duties as assigned.
