About The Position

Avail Health is seeking a Contract PRN Mobile Nurse Practitioner for the Southern Region of Indiana. This is a 1099 contract position supporting a partnership with an Institutional Special Needs Plan (I-SNP) Medicare Advantage health plan. As a mobile Nurse Practitioner, you will conduct in-person Chronic Disease Assessments and Health Risk Assessments in long-term care facilities. These assessments will support accurate clinical documentation of chronic conditions and help develop Individualized Care Plan recommendations. This role is highly collaborative, patient-facing, and requires strong clinical judgment, effective communication, and the ability to work independently in the field. Flexibility and a willingness to contribute to continuous improvement are essential in this dynamic, growth-oriented environment.

Requirements

  • Master’s or Doctoral degree in Nursing (MSN or DNP) from an accredited program.
  • Current national certification as a Nurse Practitioner (FNP or Adult-Gerontology NP).
  • Active, unrestricted Nurse Practitioner and Registered Nurse licensure in the state(s) of practice.
  • Minimum 3+ years of Nurse Practitioner experience working with older adults and/or chronic disease populations.
  • Valid driver’s license.
  • Reliable transportation and ability to travel within the assigned region.
  • Active auto insurance with proof of insurance upon request.
  • Ability to meet credentialing, privileging, and background check requirements.
  • Compliance with all applicable federal, state, and organizational regulations governing clinical practice.

Responsibilities

  • Conduct in-person Chronic Disease Assessments using historical data and in-person evaluation.
  • Support accurate clinical documentation of chronic conditions for health plan members.
  • Complete Health Risk Assessments with members and their authorized representatives.
  • Develop Individualized Care Plan recommendations (problem, goal, interventions).
  • Collaborate with Avail Health operations for scheduling, documentation, and program execution.
  • Work independently in the field.
  • Contribute ideas and feedback for continuous improvement.
