The center provides Uber/Lyft to those who don’t drive. Also, they offer exercise, art and other activities for their seniors to promote better health at no cost to the patient. This role is in Primary Care – Outpatient, utilizing a Value Based Care Model, focusing on seniors with 3-5 complex diseases (Heart disease, diabetes, hypertension etc.). The average patients per day is 10-12 with a maximum panel size of 650. Shifts are Monday-Friday 8a-5p. Nurse Practitioners have their own care team with MA/scheduler/referral specialist and full support from leadership.
Requirements
- Nurse Practitioner, board certified
- 2+ years of Primary Care Outpatient Experience
- Geriatrics experience
- Current, unrestricted, active license to practice medicine
- Current DEA registration
- Proficient PC skills in office procedures: suturing, joint injections, skin biopsies, basic wound care
Responsibilities
- Provide primary care to seniors with complex diseases.
- Manage patient panels of up to 650 individuals.
- Perform office procedures including suturing, joint injections, skin biopsies, and basic wound care.
- Adhere to a Value Based Care Model.
- Participate in a call schedule based on market provider numbers.
- Collaborate with a dedicated care team including MA, scheduler, and referral specialist.
