About The Position

WES at Philly Home at Girard and the Riverview Wellness Village provides outpatient behavioral health care within shelters and recovery housing in Philadelphia. This program, part of the Pathways to Wellness initiative, offers services like care coordination, case management, individual and group therapy, and psychiatric care for individuals with a history of substance use. The program's philosophy is to 'meet people where they are,' ensuring convenient and consistent access to behavioral health services within the residents' living spaces. The multidisciplinary team collaborates with various support providers to offer integrated care. All team members are expected to uphold principles of Diversity, Equity, and Inclusion, promoting total wellness and high-quality behavioral health services to diverse populations through compassion, innovation, and engagement.

Requirements

  • NP certification in PA with Psychiatric clinical rotation experience.
  • Minimum of one year of experience with Adults.
  • Current DEA license required.
  • Current CPR certification required.
  • Current, working knowledge of information and techniques utilized to diagnose and treat diseases, withdrawal management, and psychiatric disorders, including symptoms, treatment alternatives, drug properties and interactions, and preventive mental health-care measures.
  • Strong active listening, oral comprehension, and communication skills.
  • Ability to document care in Electronic Health Record (EHR).
  • Must meet all Department of Health (DOH), Department of Behavioral Health & Intellectual Disability Services (DBHIDS), and Community Behavioral Health (CBH) staffing requirements.

Nice To Haves

  • Experience in providing care to those with a history of substance use preferred.
  • Bilingual capabilities a plus.

Responsibilities

  • Provide psychiatric evaluations and medication management under the supervision of the Program Director and Medical Director.
  • Coordinate mental health care with interdisciplinary teams at Philly Home and Riverview Wellness Village.
  • Consult with the WES Medical Director, Program Director, and staff regarding new admissions, individualized treatment/care plans, and quality of care.
  • Ensure initial psychiatric evaluations are completed within the timeframe directed per admission policy and co-signed by the Program Psychiatrist.
  • Provide ongoing psychiatric evaluation and medication management support as medically appropriate.
  • Confer with Medical Director and staff regularly regarding clinical techniques, resident progress, and treatment.
  • Attend regular case conferences with team members for consultation and input related to resident needs.
  • Ensure documentation of progress notes and evaluations are in full compliance with clinic policies, federal and state laws, and regulations.
  • Treat residents, fellow employees, campus providers, and visitors courteously and with respect.
  • Perform other duties and special projects as assigned.
