About The Position

Ultra Health Providers is seeking a compassionate Nurse Practitioner (NP) to provide high-quality primary and preventive care to residents in skilled nursing and assisted living communities. This role offers a part-time, Monday-Friday schedule, a supportive clinical team, and the opportunity to make a difference in the lives of seniors. The Nurse Practitioner will serve as a trusted clinician, managing acute and chronic conditions, providing preventive care, and partnering with nursing staff and administrators to ensure exceptional patient outcomes. Requirements

• Active and unrestricted Nurse Practitioner license in Tennessee
• Current national board certification (AANP or ANCC)
• Minimum 1–2 years of experience in primary care, geriatric, or long-term care settings
• Strong clinical judgment and communication skills
• Commitment to compassionate, patient-centered care

Responsibilities

• Provide primary, urgent, and preventive care for residents in assigned skilled nursing facilities, and potentially assisted living facilities.
• Verify admission orders and conduct initial assessments of residents.
• Conduct physical exams, medication reviews, and depression screenings.
• Collaborate with the facility Medical Director, attending physicians, and nursing staff to deliver the highest level of care.
• Communicate with and counsel family members on treatment options when there are material changes in status.
• Manage clinical documentation accurately and in compliance with facility and state standards.
