The Nurse Practitioner (NP) provides high-quality, comprehensive primary care services to a diverse patient population within LifeLong Medical Care’s integrated team-based care model. The NP is responsible for assessment, diagnosis, treatment, and follow-up for a broad range of health conditions and preventive needs, working in close collaboration with physicians, behavioral health providers, nurses, case managers, and other members of the care team. The ideal candidate is committed to serving underserved communities and has experience in primary care, chronic disease management, and culturally responsive care. This position may report to a Site Medical Director or Associate Medical Director, depending on location. This position is represented by UAPD. Salaries and benefits are set by a collective bargaining agreement (CBA), and an employee in this position must remain a member in good standing of UAPD, as defined in the CBA.
