Nurse Practitioner
About The Position
One of a network of provincial community health centres, the NorWest Community Health Centres is a community based, non-profit organization funded by Ontario Health, emphasizing access to primary health care and prevention programs and services. We are currently seeking 1 FTE contract Nurse Practitioner to join our Homelessness and Addiction Recovery Treatment Hub (HART Hub) who will be responsible for working with an interdisciplinary team to provide a full primary care scope of practice to clients of the HART Hub. There is a strong practice emphasis on disease prevention, health promotion, health education, and community partnerships. HART Hub is a state-of-the-art treatment hub designed to provide comprehensive support and resources that address the root cause of homelessness and addiction.
Requirements
- BScN in Nursing from a recognized university and in addition a certificate in the extended class from the College of Nurses of Ontario (CNO).
- Thorough knowledge and proficiency in current extended class nursing procedures, physical assessment and treatment measures within the RN-EC scope of practice.
- Strong understanding and alignment with the concepts of trauma-informed care and culturally safer care.
Nice To Haves
- Experience in a community-based primary health care setting, addictions, homelessness, and/or mental health is an asset.
Responsibilities

- Navigating complex case management situations.
- Advocating for clients effectively.
- Creating individualized care plans.
