About The Position

The NorWest Community Health Centers, a provincial community health center funded by Ontario Health, is seeking a casual Nurse Practitioner (Registered Nurse - Extended Class). As a valuable member of the Community Health Centre team, the Nurse Practitioner will provide primary care to clients and activate pathways, referring clients for further consultation and assessments, and wrap around supports for their Gender Affirming Care Needs.

Requirements

  • 3 to 5 years of experience as a Nurse Practitioner, with experience with Gender Affirming Care provision
  • Bachelor of Science in nursing from a recognized University
  • Certificate in the Extended Class from the College of Nurses of Ontario
  • 3 to 5 years work experience in a combination of remote rural, hospital and community settings and/or public health along with Nurse Practitioner providing care to individuals who face multiple barriers to care
  • Awareness and sensitivity to socio-economic and cultural client needs
  • Awareness of and sensitivity to health issues particularly within populations who face barriers to accessing care (under housed, living with mental health challenges or diagnoses, current or past substance use, complex health histories, 2SLGBTQIA+, newcomers, non-insured)
  • Demonstrated commitment to and knowledge of community-based health care
  • Awareness of northern community health care needs
  • Demonstrated critical thinking and problem-solving skills
  • Organizational and interpersonal skills with an ability to work effectively and respectfully in a collaborative, inter-professional team environment
  • Demonstrated ability to effectively manage care needs in a professional manner
  • Superior communication (verbal and written), interpersonal, and conflict resolution skills
  • Proficiency with Microsoft Office applications (e.g., Word, Excel, Outlook)
  • Valid Ontario Driver's license
  • Access to a reliable vehicle
  • Full Covid-19 vaccination verification required, subject to any contraindications that are medically supported
