About The Position

Honor Community Health is a 501c3 Federally Qualified Health Center with 20 locations, dedicated to serving the health and wellness needs of the underserved in Oakland County. Our mission is to provide comprehensive, integrated primary, behavioral health, and dental care to all individuals, regardless of their ability to pay. The Nurse Practitioner (NP) is a key member of a high-performing integrated healthcare team, reporting to the Chief Medical Officer (CMO). The NP is responsible for managing health problems and delivering high-quality primary medical care in collaboration with the integrated practice team. This role supports the mission of providing comprehensive integrated care to vulnerable populations in Oakland County, adhering to State and Federal regulations and nursing standards of care. The NP will contribute to a culture of excellence and dedication to compassionate, high-quality healthcare. This position is for our school-based program, with site-specific start times between 7:00 am - 7:30 am, requiring a minimum 8-hour shift and participation in an on-call schedule.

Requirements

  • Graduate of an accredited Nurse Practitioner program.
  • Current Nurse Practitioner certification in the Family or Pediatric specialty area.
  • Current and valid license to practice medicine issued by the State of Michigan.
  • Current BLS and ACLS certifications through the American Heart Association, only.
  • Board certified at hire and expected to maintain appropriate board certification, licensure, and membership in professional organizations.
  • Eligible to obtain and maintain credentials as a Medicaid and Medicare provider.
  • 1+ years of progressive experience as a Nurse Practitioner.
  • Experience with CPT and ICD coding.
  • Knowledge of and experience with electronic health records (EHR) systems.
  • A compassionate and positive attitude.
  • Ability to work in a fast-paced environment.
  • Ability to create an excellent patient experience.
  • Patient focused mindset.
  • Willing to work between other locations when needed.
  • Selected candidate will be required to submit to a criminal record check and reference check.

  • Working between other locations when needed.
