A premier Life Plan Community is looking to hire a Nurse Practitioner / NP to provide high-quality, relationship-driven geriatric care within its collaborative on-campus clinical team in the Atlanta, Georgia area! This role offers the opportunity to make a meaningful impact within the community by providing holistic, relationship-driven care to older adults. The company fosters a collaborative care environment where you will work alongside geriatric physicians, nursing teams, and wellness professionals in a highly collaborative, team-based care model. You will provide meaningful, relationship-driven care across independent living, assisted living, memory care, and skilled nursing settings. The practice prioritizes personalized care, wellness, and quality of life for older adults on a beautiful 50+ acre campus with a strong reputation for excellence, offering a supportive and engaging environment for both residents and clinical staff.
Requirements
- Board Certified Nurse Practitioner/APRN/NP
- 1+ year of experience working with geriatric patients
- GA licensed
Responsibilities
- Manage patients living in the Senior living community in a clinic setting.
- See 10-12 patients per day for primary care and walk-in appointments.
- Collaborate with a Physician and Medical Director.
- Serve as the lead provider in the clinic.
