About The Position

Conduct in-home Health Risk Assessments (HRAs) for Medicare Advantage members in a flexible PRN role. The position focuses on assessment and preventive care education, without ongoing treatment responsibilities. Nurse Practitioners work autonomously, travel locally to patients in their county and nearby areas, and use a company-provided iPad and proprietary EMR system for documentation. No prescribing, on-call shifts, or follow-ups are required. Malpractice insurance and collaborating physicians are included. The role is offered as a 1099 contractor opportunity, with onboarding that may take 3 to 4 weeks.

Requirements

• Active Nurse Practitioner license; Pediatrics, PMHNP, and Women’s Health specialties are excluded.
• Availability for a minimum of eight full 7.5-hour days per month.
• Willingness to travel within roughly 60 minutes of home.

Responsibilities

• Complete comprehensive in-home Health Risk Assessments for Medicare Advantage patients.
• Review patients’ medical histories, current medications, lifestyles, and key social factors.
• Conduct basic physical and mental health evaluations.
• Provide preventive care education without ongoing management.
• Travel locally to patients within the clinician’s county and nearby areas.
