About The Position

As a Nurse Practitioner at GuideStar Eldercare, you will provide behavioral health-centered care to older adults with dementia, Alzheimer’s disease, and other neurocognitive disorders in nursing homes and assisted living facilities. The role offers the autonomy of private practice while providing an assigned caseload, collaboration with corporate clinical leadership, and administrative support from in-house scheduling, coding, and billing departments. You will work as part of a multidisciplinary team focused on patient safety, functionality, and dignity.

Requirements

Proof of state licensure in Minnesota.
Pass a pre-employment background check and drug screening.
Complete the credentialing process.
Vaccination against COVID-19 and flu is required for all GuideStar employees, subject to applicable laws.

Responsibilities

Travel daily from home to assigned facilities.
Provide initial evaluations, continuing therapy services, and cognitive testing.
Document the need for and extent of services for each patient.
Create and maintain positive professional relationships with client facilities, patients, and their families.
